I agree to hold harmless and defend The Center Helping Obesity In Children End Successfully, Inc. (C.H.O.I.C.E.S.) against any and all claims for damages, compensation, or otherwise on the part of me, my child(ren) or any other party, growing out of or resulting from injury to me, my child(ren) or any other party while participating in the activities, and to reimburse or make good any loss or damage or costs that C.H.O.I.C.E.S. may have to pay if litigation arises from injury to me, my child(ren). I hereby waive any and all rights of exemption, both as to real or personal property, to which I may be entitled under the laws of this or any other state as against claims for reimbursement or indemnity by C.H.O.I.C.E.S. 

 

I agree to follow posted rules as it relates to participating in the Event.  I am aware that the Event activities may include sample food tasting. I understand that it is my, and/or my child’s responsibility to avoid items that have been known to cause an adverse reaction. 

 

I am aware that the Event activities may include food donation. I agree to hold harmless and defend The Center Helping Obesity In Children End Successfully, Inc., (C.H.O.I.C.E.S.) if any food is given with past sale dates, expired, or held out of temperature. I understand that this food was donated and cannot be sold or exchanged for goods or services. 

 

I hereby authorize to have photographs, audio tapes, video tapes, or films taken of me and/or my child(ren). These photographs or films may be used for educational, training or publicity purposes without notification and/or compensation to me and/or my child(ren) of any kind.
